In most cases 2 vials of antivenin are enough based on the average amount of venom injected by this snake.
In some cases it happened that this dose was insufficient to reverse coagulopathy associated with the bites of this venomous snake so one more dose should be considered if there is evidence of severe envenomation (multiple bites, rapidly progressive symptoms, large snake).
Also the dose of antivenom for children should not be reduced according to their weight, since the amount of venom injected by the snake is independent of the victim’s size.
I have seen many green mambas in my garden in The Gambia. Can you please tell me if one anti venom kit is sufficient for one bite or more? Do the kits come with ull easily understood instructions. I note on your website the instructions for Hemato Polyvalent antvenin is 3 vials every six hours at a rate of 2mL/min. On the printed instructions it says “At least 20mL…should be given by slow IV infusion (not more than 1mL/min)…The second dose should be repeated 2 hours or even earlier after the initial dose. The further dose should be repeated every 6 hours according to the clinical symptoms.”

When someone is sure to work with only one snake specie it is always better to have the specific antivenin but in case someone works with different snakes our polyvalent antivenin is the best choice to avoid having to carry too many serums.

I have a couple of questions re the cobra/king cobra antivenin before I buy – We live in Aurora Provence in the Philippines and are 90 minutes from the nearest hospital that could provide real support therefore, whilst the drip/antivenin could be administered during transportation, the chances of immediate adrenalin and respiratory support if required are negligible:
a] What % of patients could expect to require respiratory support;
b] What % of patients could expect anapalactic shock to be a concern;
c] Is the storage requirement of <25 celcius critical ie. can the antivenin be exposed to temperatures higher than this for say a week as could be expected during delivery;
d] Is a single order sufficient if the victim requires more than the initial dose; and
e] What is the current thinking on whether or not a restriction bandage would be beneficial and, if so, should this remain on whilst the drip is being administered.

In some cases it happened that this dose was insufficient to reverse coagulopathy associated with the bites of this venomous snake so one more dose should be considered if there is evidence of severe envenomation (multiple bites, rapidly progressive symptoms, large snake).
Also the dose of antivenom for children should not be reduced according to their weight, since the amount of venom injected by the snake is independent of the victim’s size.

I note on your website the instructions for Hemato Polyvalent antvenin is 3 vials every six hours at a rate of 2mL/min. On the printed instructions it says “At least 20mL…should be given by slow IV infusion (not more than 1mL/min)…The second dose should be repeated 2 hours or even earlier after the initial dose. The further dose should be repeated every 6 hours according to the clinical symptoms.”
Are you able to explain the differences in the instructions please and exactly which instructions we should follow?
The second issue: are you able to use your contacts to recommend venom detection kits for our purchase? In particular I am interested in venom detection for King Cobra and for Philippine Spitting Cobra, although if you have information on the Philippine Pit Viper I would appreciate that too. Perhaps the Thai Red Cross would have this information?

I am writing to inquiry about the effectiveness of the African Snakes Polyvalent Antivenom after exposure of nearly 30 days to potentially excessive heat (was strongly stored in a container in South Sudan).There was, however, clearly no exposure to sunlight.
It has to be noted that the antivenom was not reconstituted ; the dry freeze antivenom and the 10 ml are still in separate boxes.
Could you advise on whether it is still safe to use (it is currently stored between 2-8 degrees) ? Is the shelf life diminished due to this exposure to heat ?

It’s difficult to know in advance which side effects you will have by using antivenom but it can include rash, itching, wheezing, rapid heart rate, fever, and body aches. These side effects actually are nothing compared to venom effects that can kill you.
When bitten by a snake our antivenom will fight the venom in your body but you will need to go to hospital to complete the treatment.

I have a couple of questions re the cobra/king cobra antivenin before I buy – We live in Aurora Provence in the Philippines and are 90 minutes from the nearest hospital that could provide real support therefore, whilst the drip/antivenin could be administered during transportation, the chances of immediate adrenalin and respiratory support if required are negligible:
a] What % of patients could expect to require respiratory support;
b] What % of patients could expect anapalactic shock to be a concern;
c] Is the storage requirement of <25 celcius critical ie. can the antivenin be exposed to temperatures higher than this for say a week as could be expected during delivery;
d] Is a single order sufficient if the victim requires more than the initial dose; and
e] What is the current thinking on whether or not a restriction bandage would be beneficial and, if so, should this remain on whilst the drip is being administered.
